The iMedConsent™ application enhances the education, discussion and documentation of informed consent and other patient communication processes. It is a library of procedure-specific informed consent, patient education and instructions documents, plus a software program for customizing and delivering these materials. Healthcare providers can use the system to print customized documents or to execute and store signed materials electronically.
Informed Consent Overview
Informed consent is a legal and ethical requirement in which physicians and patients exchange information concerning a patient’s condition and proposed treatment options. The process culminates in the patient’s agreement to a specific treatment or procedure. Numerous guidelines exist for informed consent including:
- The American College of Surgeons Statement of Principles for Informed Consent
- The Centers for Medicare & Medicaid Services Interpretive Guidelines for Informed Consent
- The Joint Commission (Hospital Accreditation Standard RI.01.03.01)
Survey of ACS Members
More than 97 percent of ACS members surveyed are requesting assistance with the development of informed consent templates (see survey). Overview of the iMedConsent™ PE Application Integrated into the ACS Template
The special “ACS template” of the web-based iMedConsent™ PE application was previewed at the 2009 Clinical Congress in Chicago and formally launched at the 2010 Clinical Congress in Washington, DC. This version features:
- A comprehensive template consent form contains:
- Procedure-specific consent information that supports ACS informed consent policy (including a discussion of pre- and post-surgery expectations)
- Medical record note
- Pre-procedure instructions
- Post-procedure instructions
- Forms supporting the WHO Surgical Safety Checklist (Sign In, Time Out, Sign Out)
- Detailed, easy-to-understand, procedure-specific consent forms covering all procedures and treatments for a given surgical specialty
- Each consent document has been reviewed by Dialog Medical’s Medical Advisory Board – a panel of three-board-certified physicians representing that specialty.
- Consent documents can be readily modified to reflect the singular requirements of an individual patient.
- Surgeons may also perform one-time customizations of individual consent documents to reflect their preferences or unique approach to a particular procedure. Those modifications will “stick” for that surgeon but will not impact the customizations made by other physicians within the same practice.
- Exhaustive content library of patient education materials, anatomical images, and drug monographs in English and Spanish for a single specialty.
- All clinical content is updated continuously throughout the year.

Sample Documents
Dialog Medical’s Customer Service Team will assist you with customizing your informed consent template to meet the needs of your practice. Customizations might include adding a particular legal statement or adding language concerning observers, photography, or tissue samples. Four typical informed consent templates are listed below:
CVC Catheter Placement Consent
Employs a detailed consent form containing embedded pre- and post- instructions.
Chest Tube Placement Consent
Employs a detailed consent with separate pre- and post- instructions.
Appendectomy Consent
Employs a detailed consent with a medical record note and pre- and post- instructions.
Laparoscopic Cholecystectomy Consent
Employs a detailed consent, a medical record note, pre- and post- instructions, and the WHO Surgical Safety Checklist.
Getting Started
There are two options for experiencing the iMedConsent™ PE application. First, you can create a User Name and Password to a limited, demonstration version of the iMedConsent™ PE application. All you need is your ACS Member Number to be up and running in 60 seconds. This demonstration version of the iMedConsent™ PE application will allow you to do the following:
- View sample consent documents in the Consents – Samples folder.
- View the titles of the consent forms and other documents available to your practice.
- Please note that the only documents that can be viewed in their entirety are the consent documents in the Consents – Samples folder.

The second option for experiencing the iMedConsent™ PE application, available exclusively to ACS Members, is a free 30-day evaluation.
